For your convenience, our patient forms are available for you to download. Please complete the 2 forms (Patient Registration and Medical History Form) prior to your first appointment. Your completed forms will be sent securely to our office.
If your first visit is for a dental emergency, only the tooth in question will be addressed. Depending on the diagnosis, an additional appointment with adequate time may be necessary.
